How to Apply
MEDICAL, VISION & PRESCRIPTION
Employees have a choice between two Regence medical plans, either a $500 or $1000 deductible. The out-of-pocket max also differs between the two plans, otherwise the coverage is the same.
Employees have the choice between Moda/Delta Dental and Willamette Dental. Moda/Delta Dental offers an incentive plan (coverage increases each year, as long as an exam is done each year), where Willamette Dental has a copay system. Willamette includes limited orthodontia coverage.
Employer-funded Health Reimbursement Account. The amount funded monthly depends on the employee’s medical plan enrollment.
SHORT TERM DISABILITY
13 weeks of leave paid at 60% of the employee’s wages up to $900/week.
LONG TERM DISABILITY
Benefits begin when Short Term Disability ends (90 days) and pay 60% of earnings up to $5,000 a month. A claim can last up to age 65.
A $20,000 life insurance and accidental death and dismemberment policy automatically issued (no health screening required).
The table below illustrates the Paid Time Off benefits for employees based on years of service. Accruals occur at the hourly rate and occur every-other week, and are based on the employee’s anniversary date.
For new hires, some PTO is “front loaded” upon starting work. The front-loaded amount is not payable if employment resigns or is terminated (for any reason) within the first 6 months.
Employees receive 8-hours of regular pay for each of 11 holidays off: New Year’s Day; MLK Jr. Day; Presidents’ Day; Memorial Day; Independence Day; Labor Day; Veterans Day; Thanksgiving Break (2 days); Christmas Eve; Christmas Day.
Employees receive 3 days of paid leave for the death of a covered family member, as defined by Oregon Family Leave Act (OFLA). Additional time off is available to the employee using their PTO bank or unpaid leave.
The following benefits are available to employees at their discretion: Flexible Spending Account (FSA); Dependent Care Reimbursement Account (DCRA); AFLAC; Wellness Benefit ($15/month post-tax); and, Deferred Compensation. All of these are employee funded with the exception of the Wellness Benefit.
COST OF BENEFITS TO THE EMPLOYEE:
Each employee contributes only 1.5% of their income toward benefits.
